GLP-1 Patches Review 2026: Semaglutide Is 4,114 Daltons. Skin Stops at 500.

Most reviews of weight-loss patches argue about whether the ingredients work. That argument is unnecessary. There is a published rule for what can cross human skin, the molecules in these patches are measurably on the wrong side of it, and you can check the arithmetic yourself.

Search for a GLP-1 patch and you will find a large and growing market: berberine patches, appetite-suppressant patches, metabolism patches, "natural GLP-1" patches, moringa microdarts, and dedicated collections from established vitamin-patch brands. PatchAid sells a GLP-1 Support collection — patches from $14.97, packs from $35.97 to $59.97 — and says they "help control appetite, boost metabolism, and ease nausea" for people using therapies like Ozempic and Wegovy. We reviewed the category on August 2, 2026. We have no affiliate relationship with any patch brand mentioned here. This review is unpaid. Most coverage of these products gets bogged down arguing about the ingredients — does berberine work, does chromium do anything, is there evidence for moringa. Those are interesting questions and they are, for this purpose, beside the point. Because before an ingredient can do anything, it has to get into you. And there is a published, well-established rule about what can cross human skin. It has a number in it. 500 Daltons. Everything below follows from comparing that number to the molecules being sold.

The 500 Dalton Rule

In 2000, Jan Bos and Marcus Meinardi published a paper in *Experimental Dermatology* titled "The 500 Dalton rule for the skin penetration of chemical compounds and drugs." It is one of the most cited papers in dermatological pharmacology, and its conclusion is a single sentence: "The molecular weight of a compound must be under 500 Dalton to allow skin absorption." The authors gave three independent lines of evidence, and the elegance of the argument is that they converge from completely different directions: One — contact allergy. Virtually every common contact allergen is under 500 Daltons. Larger molecules are not known as contact sensitizers. They cannot get through the skin to reach the immune cells that would react to them. Two — topical dermatology. The pharmacological agents actually used in topical treatment are all under 500 Daltons. Three — transdermal patches. Every drug successfully used in a transdermal delivery system is under 500 Daltons. That third point is the one to hold onto. Transdermal patches are a real and successful drug delivery technology. They are also a narrow one. Look at what is actually on the market:

Notice what every one of these has in common beyond size. They are lipophilic — they dissolve into the fatty matrix of the stratum corneum. And they are potent, meaning a therapeutic dose is measured in micrograms or single-digit milligrams per day. A transdermal system that has to deliver hundreds of milligrams is not a design problem. It is a non-starter.

Now Weigh the Molecules

Start with the top two lines, because they settle the headline question completely. Semaglutide is a 4,114-Dalton peptide. Tirzepatide is 4,814. They are eight to ten times past the threshold for passive skin absorption, and they are peptides — which skin enzymes degrade even where they do manage to penetrate. There is no FDA-approved transdermal GLP-1 medication, and this is why. Any product whose marketing implies you are getting a GLP-1 drug through a sticker is not describing something difficult. It is describing something that does not happen.

Why Small Enough Still Isn't Enough

Three of the substances above are under 500 Daltons, and it would be unfair not to take that seriously. Size is necessary but not sufficient, and the other two criteria do most of the remaining work. Berberine — small enough, and permanently charged. At about 336 Daltons, berberine clears the size bar. But it is a quaternary ammonium cation, meaning it carries a permanent positive charge. Charged molecules do not partition into the lipid matrix of the stratum corneum; that barrier is fatty, and it repels ions. That alone makes passive transdermal delivery implausible. Then there is the dose. Berberine's studied oral dose is 500 mg two or three times daily — 1,000 to 1,500 mg a day — and that is *because* its oral bioavailability is under 1%, so you need a large amount to get a small amount in. For comparison, a nicotine patch, one of the highest-output transdermal systems in routine use, delivers roughly 7 to 21 mg over 24 hours. A berberine patch would need to deliver something on the order of a hundred times the output of a nicotine patch, through skin, using a permanently charged molecule. That is not a marginal claim. It is off by two orders of magnitude. Glutathione — small enough, and too water-loving. At 307 Daltons it clears the size bar, but it is highly hydrophilic and ionised at skin pH, which is the same partitioning problem in a different form. Vitamin D3 — the honest exception. At roughly 385 Daltons and extremely lipophilic, cholecalciferol is the one ingredient in this category with a genuinely plausible physicochemical profile, and it is needed in micrograms rather than milligrams. We are not going to tell you a D3 patch cannot work on first principles, because the first principles do not say that. What we will say is that plausible is not the same as demonstrated, and we found no clinical data showing a patch raises serum 25-hydroxyvitamin D. If you are deficient, the way to find out whether anything worked is a blood test, which costs less than a year of patches.

This gives you a three-question test you can apply to any transdermal product for the rest of your life, without needing to know anything about the ingredient. Is the molecule under 500 Daltons? Is it fat-soluble and uncharged? Is the daily dose small enough to plausibly cross a few square inches of skin — micrograms to single-digit milligrams? A product needs all three. Most patches in the wellness category fail the first or the third, and many fail both.

What the Sellers Say Instead

Given all that, the interesting question becomes what these products claim about how they work. Here is PatchAid's explanation, which is representative of the category: The patches are described as having a formula layer containing the vitamins, a contact layer activated by body heat, a protective backing layer, and an adhesive layer that touches the skin. Read that again. It is a description of how a sticker is built. It tells you the ingredients are in there, that there is a backing so they do not go the wrong way, and that there is glue so it stays on. It does not describe any mechanism by which a molecule crosses the stratum corneum — no penetration enhancer, no microneedle array, no iontophoresis, no reference to any of the actual technologies that exist for pushing difficult molecules through skin. "Activated by body heat" is not a delivery mechanism; warmth does not make a 1,355-Dalton molecule smaller. We also found no scientific studies cited anywhere in the collection. The site carries the standard notice that its statements "have not been evaluated by the Food and Drug Administration." This is worth separating from the harsher accusations sometimes thrown at this category. These products almost certainly contain what they say they contain. Nobody is being poisoned. The claim under examination is not whether the vitamins are in the patch. It is whether the vitamins get out of the patch and into you, and on that question the sellers have offered a diagram of an adhesive bandage. There is also precedent for regulators treating this as an advertising problem rather than a scientific debate. In 2007 the Federal Trade Commission fined Transdermal Products International Marketing Corporation and its owner $180,000 and banned further sales, finding that "advertising claims for their weight-loss patches were false and unsubstantiated." The FTC's guidelines for health-related products require advertisers using studies to disclose how the dosage and formulation of the advertised product compare with the studied one, whether the ingredients are the same, and whether the product is administered the same way. Reporting by PBS NewsHour found a patch brand citing studies on berberine, glutamine and chromium in which the ingredients had been taken orally — while the product being sold was a patch.

It May Not Legally Be a Supplement At All

A regulatory wrinkle worth knowing, because it changes what protections you have. Under the FDA's definition, a dietary supplement must be swallowed. As Jensen Jose, regulatory counsel at the Center for Science in the Public Interest, put it to PBS NewsHour: because patches are not swallowed, they are not supplements — and it is not clear whether they would be classified as a cosmetic, a drug, or possibly both. That matters. The supplement framework created by the Dietary Supplement Health and Education Act is already permissive: manufacturers must tell the FDA they hold evidence for their claims, but as Jose notes, they do not actually have to produce it. A product sitting outside even that framework is in a stranger place still. Two more expert observations from the same reporting are worth carrying with you: C. Michael White, professor of pharmacy practice at the University of Connecticut School of Pharmacy, on the marketing: *"The marketing seems to suggest that you could get with these natural products... the same effect that you can get with GLP-1 agonists, which is simply not the case. And in that way, they are being misleading to people."* He added that some products "don't even tell you what ingredients are in it at all." Caitlin Dow, nutrition scientist at the Center for Science in the Public Interest, on the evidence: companies routinely fail to cite any study in which all the major ingredients were tested together — which matters, because ingredients can work against one another. And a caution that cuts against the assumption that a patch is automatically the gentler option. White noted that if a transdermal route did increase bioavailability, the consequences would not all be good: some ingredients could interfere with drugs a person takes for diabetes or heart disease. "It may be more dangerous to be taking the products in this way than it would be to be taking them orally."

The scale here is worth registering, because it is not a fringe product. According to publicly available TikTok Shop figures reported by PBS NewsHour, one "GLP-1 Patches" product sold for around $15 had moved more than 250,000 packs, carried a four-star average across 9,100 reviews, and was the top-selling item in its category on the day it was checked. Affiliates earned $2.97 per 30-pack sale. That review average reflects an affiliate-driven sales channel, not evidence of transdermal absorption.

White's practical advice for anyone buying any supplement is the same one we would give: look for products verified or certified by U.S. Pharmacopeia (USP), NSF or ConsumerLab, which test for adulterants and heavy metals. The One Place This Becomes Genuinely Dangerous

Most of what we have described costs money and does nothing. That is a bad deal, not an emergency. There is one exception, and it matters specifically to the people reading a GLP-1 site. Vitamin B12 patches, used by anyone who cannot absorb B12 orally. That population is larger than people realise, and it overlaps heavily with this audience. It includes people who have had bariatric surgery — gastric bypass and sleeve gastrectomy both impair B12 absorption, by removing or bypassing the parts of the stomach that produce intrinsic factor. It includes people with pernicious anaemia, with atrophic gastritis, with Crohn's disease, and people on long-term metformin or proton pump inhibitors. These are patients for whom B12 status is a monitored clinical parameter, usually managed with intramuscular injections or high-dose oral protocols. Cyanocobalamin is 1,355 Daltons — nearly three times the skin penetration limit — and it is a large, water-soluble molecule with a cobalt centre. A patch is not a plausible route. B12 deficiency does not announce itself. It presents slowly: fatigue, then paraesthesia, then difficulty with balance and gait, then cognitive changes. And the neurological damage from prolonged deficiency — subacute combined degeneration of the spinal cord — can become irreversible. The haematological picture can also be masked if you are taking folate, which lets the neurological damage progress silently while the blood count looks acceptable. Someone who switches from injections to a patch because it is easier and less unpleasant may feel fine for months while a preventable, permanent injury develops.

If a clinician has prescribed B12 injections or a specific oral protocol, do not substitute a patch for it, and do not let a patch delay a scheduled injection. If you have had bariatric surgery, have pernicious anaemia or malabsorption, or are on long-term metformin or a PPI, your B12 needs to be measured in blood, not assumed from a product you are wearing. The same logic applies to iron and vitamin D after bariatric surgery. This is the one part of this review that is not about wasted money.

There is a second, quieter risk worth naming: the nausea patch. Nausea is the most common side effect of starting a GLP-1 medication, and it is usually benign and self-limiting. But persistent, severe or late-onset vomiting can also be the presentation of pancreatitis or gallbladder disease, both of which are recognised risks on these drugs. Reaching for a patch and waiting it out is the wrong instinct. Severe abdominal pain radiating to the back, or vomiting that will not stop, is a same-day call to your prescriber.

What Actually Helps, for the Same Money

The demand behind these products is real. People on GLP-1 medications genuinely do have appetite, nausea, constipation and nutrient problems, and it is reasonable to look for help with them. The patch is the wrong answer to a sensible question. For roughly what a month of patches costs, here is what has evidence behind it:
  • For nutrients: an ordinary oral multivitamin, or the specific nutrient your bloodwork says you are low in. Oral is the route with the absorption data.
  • For muscle preservation: 1.2–1.6 g of protein per kg of body weight per day, plus resistance training twice a week. This is the single highest-value intervention on a GLP-1, and the one most people skip.
  • For constipation: soluble fiber and adequate fluid. Psyllium is cheap, well-evidenced and works.
  • For nausea: smaller meals, less fat, and slower dose escalation — a conversation with your prescriber about titration beats any product.
  • For B12 specifically: get a serum B12 level. If you are low, take the oral or injectable form your clinician specifies.
  • For vitamin D: get a 25-hydroxyvitamin D level, then supplement orally to target and retest. Guessing is how people end up either deficient or over-supplemented.

The theme is that every item on that list is measurable. You can check a B12 level, a vitamin D level, a scale reading, a protein intake. The patch category asks you to trust an outcome you have no way to observe — which is, not coincidentally, the ideal design for a product that does not produce one.

Request a refund if the seller offers one, and if you were relying on a B12 or vitamin D patch, get a blood level checked so you know where you actually stand.

The Bottom Line
