Ozempic Dosage Chart: Starting and Maintenance Doses

A complete guide to Ozempic dosing for diabetes and weight management. How to titrate and what dose is right for you.

By Med Consumer Watch Team
Ozempic (semaglutide) is FDA-approved for type 2 diabetes at doses up to 2mg weekly. Here's the complete dosing guide.

Ozempic Dosing Schedule

Standard titration for diabetes:
  • Weeks 1-4: 0.25mg once weekly (initiation dose, not therapeutic)
  • Week 5+: 0.5mg once weekly (first therapeutic dose)
  • Optional: 1mg once weekly (if additional control needed)
  • Optional: 2mg once weekly (maximum dose)

Dose Selection

How doses compare: • 0.5mg: Effective starting therapeutic dose for most • 1mg: More glycemic control, more side effects • 2mg: Maximum dose, approved in 2022 For diabetes, many patients do well at 0.5mg or 1mg without needing 2mg.

Ozempic's maximum approved dose (2mg) is lower than Wegovy's (2.4mg). For maximum weight loss effect, Wegovy is the appropriate product.

Pen Options

Ozempic pens: • 0.25mg/0.5mg pen (delivers both starting doses) • 1mg pen • 2mg pen Each pen contains multiple doses.

Off-Label Weight Loss Use

When used off-label for weight loss: • Same titration schedule applies • Providers may target higher doses • Results typically less than with Wegovy • Insurance coverage may differ

Ozempic is approved for diabetes, not weight loss. For weight management indication, Wegovy is the appropriate choice.

The Bottom Line

Ozempic titration starts at 0.25mg for 4 weeks, then 0.5mg as the first therapeutic dose, with options to increase to 1mg or 2mg based on response. Work with your provider to find the optimal dose for your blood sugar control.
