Zepbound (tirzepatide) is the weight loss-approved version of tirzepatide (also sold as Mounjaro for diabetes). The dosing schedule is identical to Mounjaro.
Zepbound Dosing Schedule
Standard titration:
- Weeks 1-4: 2.5mg once weekly (starting dose)
- Weeks 5-8: 5mg once weekly
- Weeks 9-12: 7.5mg once weekly
- Weeks 13-16: 10mg once weekly
- Weeks 17-20: 12.5mg once weekly
- Week 21+: 15mg once weekly (maximum dose)
Dosing Flexibility
Important considerations:
• Increase every 4 weeks OR stay at current dose if it's working
• Not everyone needs to reach 15mg
• Some patients respond well at lower doses
• Slower titration may reduce side effects
Work with your provider to find your optimal dose. The goal is effective weight loss with manageable side effects, not necessarily the maximum dose.

Zepbound single-dose pens:
• 2.5mg
• 5mg
• 7.5mg
• 10mg
• 12.5mg
• 15mg
Missed Dose Instructions
If you miss your weekly injection:
• Within 4 days of missed dose: Take it immediately
• More than 4 days: Skip and continue regular schedule
• Never take two doses within 3 days of each other
The Bottom Line
Zepbound follows the same titration as Mounjaro—20+ weeks to reach maximum dose. The gradual approach minimizes side effects and helps identify your optimal therapeutic dose.
