Ovyvo sells medical care and lets the medication be bought wherever it is cheapest, which inverts the model almost every competitor uses. Our research (September 2, 2026) found a practice whose commercial structure is unusually aligned with the patient's interest.

The Fee Is for the Doctor, Not the Drug

Separating the care fee from the medication cost matters more than it sounds. When a provider's revenue comes from dispensing a compounded drug, every clinical decision runs against a commercial pull toward that drug. When the revenue is the visit fee, the provider can recommend whatever is actually best — including a brand-name product they make nothing on, or no medication at all.

Brand-Name Is a Real Option, Not a Footnote

Compounded medication is asterisked throughout rather than presented as equivalent, and the ordering — brand-name first — reflects the clinical hierarchy correctly. A patient who can access Wegovy or Zepbound through insurance is helped to do so. Where compounded product is used, the standard caveats apply: not FDA-approved, not reviewed for safety, effectiveness or manufacturing quality, and following resolution of the tirzepatide shortage in October 2024 and semaglutide in early 2025, lawful 503A compounding requires a documented clinical reason for the individual patient.

An EKG at Intake

The initial visit includes review of labs and an EKG alongside health history, goals and treatment options. Baseline cardiac and metabolic workup before starting a GLP-1 is more than most telehealth operators do — many require no laboratory work at all — and it is the kind of thing that distinguishes a medical practice from a fulfilment pipeline. The honest trade-off: $125/month buys no medication. A patient paying $125 in care fees plus, say, $200 for compounded tirzepatide is at $325 — more than several all-in competitors. The structure is better aligned and clinically deeper, but it is not automatically cheaper, and patients should total both components before comparing.
