Rivas Medical Weight Loss Tirzepatide Review

Rivas is a brick-and-mortar weight loss practice that publishes its prices — a combination we have not often found. Our research (August 8, 2026) found a model built around weekly in-person contact rather than a shipped subscription.

The Pricing Is Actually Published

Most of the operators in this comparison bury pricing behind an intake quiz. Rivas states it plainly:

• $195 initial visit — an in-person evaluation with vitals and weigh-in, which also confers lifetime membership
• $115 per week, all-inclusive, for compounded semaglutide — medication, provider visit, vitals and dose adjustment in one price, at any dose
• $165 per week for compounded tirzepatide, on the same all-inclusive basis
• $100/month follow-ups for patients on brand-name Wegovy or Zepbound, with medication purchased separately at a pharmacy
• 5% off 1-month packages and 10% off 3-month packages

The price does not change as your dose increases. That is a meaningful structural difference. Many telehealth programs raise the monthly charge at each titration step, so the cost climbs exactly as patients reach therapeutic doses. Do the arithmetic honestly, though: $115/week is roughly $498/month, and $165/week roughly $715/month. That is well above the $199-$299 typical of shipped telehealth. What the premium buys is a weekly in-person provider visit with vitals, and medication administered in-office rather than self-injected.

Weekly In-Office Care Is the Actual Product

Rivas describes the model directly: "At Rivas, you walk in every week. Your provider knows what happened last visit. The front desk knows your name." For compounded programs, the first dose is administered in-office the same day, and subsequent doses weekly. That is a genuinely different care model from a monthly vial shipped to your door — more contact, more monitoring, more friction. Whether the friction is worth $200-$400/month more depends entirely on whether weekly accountability is what has been missing for you.

Lifetime Membership

The $195 initial visit is a one-time charge. Rivas states you remain a member for life provided you visit at least once a year — "No re-enrollment, no restart fees." For anyone who has cycled on and off weight-loss programs, not paying to restart is a real benefit.

Regulatory and Payment Detail

Rivas states it uses U.S.-licensed 503A pharmacies for compounded semaglutide and flags "Licensed 503A / 503B" on its comparison material. It explicitly notes that "compounded medications are not FDA-approved finished drug products." HSA and FSA are accepted across all options, with Cherry and CareCredit financing available for in-office packages. There are no contracts and patients can cancel anytime. For brand-name prescriptions, Rivas writes the prescription and helps patients navigate pharmacy and Novo Nordisk patient programs, but is clear that medication pricing is determined at the pharmacy and not by Rivas.

Reach and Limits

15 clinic locations across Maryland and Virginia; telehealth in Maryland, Virginia and Florida only. Telehealth compounded programs ship to your door with a monthly video visit rather than weekly in-office care. If you live outside those three states, Rivas is not available to you. Add-on services include phentermine (described as an FDA-approved oral appetite suppressant, physician-prescribed and monitored at each visit) and a B-vitamin/carnitine injection blend. The B-vitamin blend is marketed for energy and metabolism support; that category of claim is not one we would rely on.
