LillyDirect Tirzepatide Review

If you have been comparing compounded GLP-1 subscriptions, this is the option that should anchor the comparison. Our research (August 8, 2026) found the manufacturer selling its own FDA-approved product directly, at published prices, with no compounding involved at all.

The Self-Pay Prices, Dose by Dose

Lilly publishes the full schedule for a one-month supply of the Zepbound single-patient-use KwikPen:

• 2.5 mg — $299 (starting dose; not an approved maintenance dose)
• 5 mg — $399
• 7.5 mg — $499
• 10 mg — $699
• 12.5 mg — $699
• 15 mg — $699

Lilly notes the $299 price applies to the KwikPen or vial, not the Zepbound single-dose Pen, and that additional taxes and fees may apply. Pen needles are required and sold separately. This is real Zepbound — the FDA-approved product that produced roughly 20.9% mean weight reduction over 72 weeks in SURMOUNT-1. Not a compounded copy. As our brand-name versus compounded guide explains, that distinction now matters more than it did during the shortage years, because the shortage-based justification for compounding tirzepatide ended when the FDA declared the shortage resolved.

How It Compares

At the low doses, compounded telehealth is still cheaper — Trimi advertises $125/month for compounded tirzepatide against Lilly's $399 at 5 mg. But the gap narrows sharply once you account for what you are getting:

• An FDA-approved finished drug product with a known manufacturer, reviewed for safety, effectiveness and manufacturing quality
• No question about which pharmacy compounded it, what pathway it operates under, or whether it was tested
• Doses that match the trial regimens exactly

And several compounded programs sit at $299-$499 anyway — WeightCare's monthly renewal is $499, and iThriveMD charges $399 for tirzepatide on a 28-day cycle. At those numbers there is no cost argument left for a compounded copy.

Foundayo: The Oral GLP-1

LillyDirect's pharmacy page also lists Foundayo™ (orforglipron), Lilly's oral GLP-1, in doses of 0.8, 2.5, 5.5, 9, 14.5 and 17.2 mg, starting at $149 per month through LillyDirect self-pay. This is worth understanding clearly, because it is a genuine oral GLP-1 rather than a compounded workaround. Throughout our reviews we have flagged sublingual drops and rapid-dissolve tablets sold with no absorption evidence. Foundayo is a different thing: a pharmaceutical product with its own development programme, dosing and label. If you want an oral option, this is the route that comes with evidence attached.

Insurance and Medicare Routes

Commercial insurance that covers Zepbound: a savings card applies to up to a 3-month prescription. Commercial insurance without Zepbound coverage: a separate savings card applies to a 1-month KwikPen prescription. Medicare: Lilly states that eligible Medicare Part D patients with a Zepbound prescription for weight management may obtain it for no more than $50/month through the Medicare GLP-1 Bridge, subject to Medicare GLP-1 Bridge Clinical Criteria. Lilly is explicit that this is not a guarantee of coverage. The exclusion to know about: the commercial savings cards specifically exclude patients enrolled in Medicaid, Medicare, Medicare Part D, Medicare Advantage, Medigap, DoD, VA, TRICARE or any state prescription assistance program. Self-pay pricing remains available to those patients, and the Medicare Bridge is the separate route.

How You Actually Get It

LillyDirect is a pharmacy, not a prescriber. You need a prescription from your own healthcare provider, sent to LillyDirect Pharmacy, after which Lilly says it guides patients through savings options, prior authorisation and checkout. If you do not have a provider, LillyDirect lists independent telehealth partners with their own published pricing — General Medicine at $59/month on an annual subscription, knownwell at $299 per visit self-pay, Form Health at $299/month, and HealthTap at $129 per visit with an annual subscription. Those are separate companies with their own fees, and using one is not required.

The Honest Limitations

$699/month at maintenance doses is a lot of money if you have no coverage. The published schedule rises steeply from 7.5 mg upward, and 10 mg and above all sit at $699. Lilly can change or end these programmes. The terms state the offer may be terminated, rescinded, revoked or amended at any time, with or without notice. It only sells Lilly products. If semaglutide is the right molecule for you, NovoCare Pharmacy is the equivalent route from Novo Nordisk.
